The St. Demetrios Greek Orthodox Community of Sarnia and Vicinity is updating the community database with a community questionnaire. The purpose of this questionnaire is to update the current St. Demetrios membership database and to collect information to help us develop a better understand of the makeup of our community.
The information collected will be kept in accordance to the Canadian Privacy Act (PIPEDA) and kept solely for the use of the ecclesiastical community of the St. Demetrios Greek Orthodox Church of Sarnia and the jurisdiction of the Greek Orthodox Metropolis of Toronto (Canada) [Uniform Community Regulations - Article I]. 
Please complete the attached questionnaire. If you prefer working on the web, a copy of this questionnaire is available at www.sarniastdemetrios.com. You may complete and submit the form online.
In order to complete this project in a timely manner, we kindly ask that you return the questionnaire by Monday, August 23, 2010. If you are aware of any resident of Sarnia Lambton who may be of Greek descent or Greek Orthodox who did not receive this questionnaire, please contact Theo so that we may include them in this project.

Instructions for St. Demetrios Greek Orthodox Community Questionnaire

Section A of the Questionnaire: is for the Father/Husband to be completed

Section B of the Questionnaire: is for the Mother/Wife to be completed

Section C [Part A] of the Questionnaire: is for children under the age of 18

Section C [Part B] of the Questionnaire: is for young adults age 18 and older who still live at home.

When you have submitted the questionnaire, we will receive an email copy - and you will receive a copy for your records too.
